Comprehensive Assessments: Thorough evaluations of your overall health, including medical history, current conditions, and special needs. After the initial assessment, information will be given to you on possible resources to address your specific needs.

Personalized Care Planning: Customized care plans tailored to your unique health goals and preferences. A personalized care plan is included with every initial assessment.  Interventions on the care plan may be things we can help with, or referrals to local resources.

Medication Management: Organization and oversight of medications to ensure proper use and prevent complications.  This may include medication reminders, pill planners, education and more.  Taking medications as ordered by your doctor is vital to staying out of the hospital and managing your chronic conditions.

Coordination of Care: Seamless coordination among all healthcare providers to ensure continuity of care.  This may include updating all of your providers of changes, providing transportation to appointments, accompanying clients to appointments  and updating approved family members.

Health Education and Advocacy: Empowering you with the knowledge and support needed to make informed healthcare decisions.  This may include disease specific education, medication education and advocating for you as you navigate the health care system.

Chronic Disease Management: Ongoing support to manage chronic conditions such as diabetes, hypertension, and heart disease. Bright Path Elder Care is able to contact your providers when there are concerns about your chronic conditions. No more waiting on hold and leaving voicemails!

Transitional Care: Assistance during transitions between different care settings, such as from hospital to home. Times of transition can be especially difficult.  Imagine you are discharged from the hospital with new medications and medical equipment.  We will ensure you have what you need to be successful at home.

Caregiver Support: Support and education for caregivers.  This may include resources specifically for the caregiver or family members.  We can provide help for family members that may live in a different state and need more eyes and ears on their loved one.

Emergency Planning: Development of emergency plans including organization of advanced directives, medical records, and other documents that you may need to access quickly in an emergency.

Conflict Resolution:  We help facilitate communication among clients, caregivers, and family members.   Sometimes a family meeting might be needed to resolve conflict.  We can provide a calm, unbiased voice during difficult times.
